Creation of index fails in eoatex2pdf.py

To create the index in a book, the file preambel/dontdeleteindex.ist is required. By copying the texfiles into output/latex, this file is not considered.

Maybe it's better to do the latex runs in the main directory of the publication and then move all the files to output/latex?

fixed in branch clean_and_fix (commit 387e952)
avoid using --output-directory option with the xelatex command, by copying all needed files to the output directory and running xelatex there.
